Please click and fill out the corresponding LOD (Letter of Direction) form below to match the Index Account you are opening:

3. Submit an ID and proof of residence.

4. Wait for the account to be approved. Our operations department will notify you
regarding the status of your account application by e-mail.

5. Finally, fund the account using one of the methods described on:
